Clark forklift side shift assembly with optional high termination quick connect fittings. This was on a 2015 Clark C25 (5000 pound lift) with 780 hours when removed. Absolutely nothing wrong with this. I replaced it with a side shift fork positioner assembly. Forks are 48” long and 4” wide class II. This is all standard size and should work on other brand trucks I believe.
Or you could also weld on the back side and turn it into a 3 point lift for your tractor as well.
